Advanced Health Resources provides services to children, adolescents and adults with a primary focus on strengths and needs to help the individual achieve their goals. We work with the person and others who are important to them to improve and/or maintain skills in areas such as: independent living, socialization, coping, activities of daily living and other areas identified in the person’s plan. Our dedicated, knowledgeable, team of professionals and support staff will assist you in planning for and accessing services and supports. We use a person first, family centered approach while helping the individual to improve their quality of life.
AHR is proud to have achieved three-year accreditation status from CARF for the following services based on National Standards:
- Community Inclusion/ Integration (HCS, DT); Respite (both Medicaid and IPRS)
- Case Management (TCM, CSS- Child and Adult)
